When I received the original form I completed it and immediately mailed it to Pennsylvania by Thailand Post with tracking. That showed the form successfully made it to USA but then no further info. They later emailed me saying they had not received it. Later I made a copy of the form and sent it to FBU Manila as instructed to by SSA by DHL. Tracked the shipment and that showed it did reach Manila. The airport immigration office has ended next day residency certificates. You now pay 60 baht and have to wait "7 business days" for it to be sent by Thailand post. Previously you could pay 500 and collect it the next day. Maybe its only temporary, but I asked the lady and she said they're not doing it now. Well so I sent some extra clothes I had no space for after getting souvenirs (no buddda statue or ladyboy), guy at post checked my address twice, I came back home and my package is at post with "wrong address" info, tried to call local post but they not answering my phone.
